My son was toilet trained but since the birth of my daughter he has started to wet his pants and his bed. His urine smells really bad. Is he lazy or doing it for attention? Or maybe it is something to do with his health?

My son was completely toilet trained, day and night. But since the birth of my daughter, almost immediatly, he started to wet his pants, and wet the bed. He is now 4 years old, it has been nearly 2 years since he started wetting his pants, and it is worse then ever. He wets his pants 8 times in a day on bad days, and on top of that, his urine smells really bad? I don't know if he does it because he is lazy, or for attention? Or maybe it is something to do with his health? Please help!!

Dr Cathrine Answered:

When a child starts to wet the bed again or experiences daytime incontinence after being dry for months we usually look for either a medical or emotional cause. One of the most common causes is stress. Almost any change in the environment – good or bad – can be a trigger. So the birth of his new sister may well have triggered his wetting although it does not fully explain why his bedwetting has continued for two years nor does it explain his daytime incontinence. We need to work on helping him achieve daytime dryness before tackling his nighttime bedwetting. It is unlikely that he is doing this out of laziness or for attention – talk to him about how he feels when he needs to go to the toilet, is he able to recognize the physical signs that he needs to go? Given the frequency of his wetting and the fact that he has strong smelling urine I would advise that you seek a full medical assessment – your doctor will probably ask you to keep a record of his fluid intake and urinary output – you could start this now so you have something to take to your first appointment.
